Position Title: In Home Service Worker

Reports To: In-Home Services Coordinator

AREAS OF CONSIDERATION

- Prefer a Washington County or Greene County resident.
- Travel will be required throughout Greene County.
- Final determination of suitability for employment will be contingent upon a successful Childline & Criminal Investigation as prescribed in the Child Protective Services Law (Act 33 & Act 34).
- Physical examination including an intradermal TB test.
- Ability to lift a minimum of 20 pounds required.
- One year experience giving personal care and/or 40 hours training in personal care required

EDUCATION

- High School Diploma or G.E.D. not required

WORK WEEK

- Days and Hours will vary depending on client need.

BRIEF DESCRIPTION OF DUTIES

1) The following list of activities will be considered in provision of this service:
a) Bathing or assistance with bathing in the tub, shower, or bed.
b) Skin care.
c) Mouth care, including care of dentures.
d) Dressing, including application of prostheses.
e) Grooming (except toenails of a diabetic client).
f) Toileting, including transfers on and off toilet facilities.
g) Ambulation and transfer.
h) Change of position or turning client.
i) Feeding, including assistance in preparing and serving.
j) Assistance with self-administered medication.
k) Instruction in delivery of care to informal care givers.
2) In Home Service may also include provision of supplemental housekeeping service as long as the primary service is from the above list. The supplemental housekeeping services include:
a) Washing dishes, cleaning up after meal preparation.
b) Making beds and linen changes.
c) Shopping.
d) Washing the client's personal laundry.
e) Light housekeeping (essential for healthful environment).
f) Assistance with simple home and money management.
3) In addition to these direct services, the In Home Service Worker will be required to observe and report any changes in client’s conditions and needs. Reports will be made in writing or with written follow-up to an oral report. Reports will be filed in the client record.
4) Records of service will be generated, compiled into reports, and submitted in a timely fashion. Records and reports will be accurate and complete.
5) Attendance at some training and in-service meetings, as required by supervisor, will be mandatory.
